WHAT IS 5 WHYS?
A simple technique for finding the root cause of a problem by repeatedly asking why — each answer opening the next question. Created by Sakichi Toyoda and popularized by Toyota, its golden rule is to keep asking why until you can't anymore: that's where the true root cause lives.

ONE PROBLEM, THREE PATHS
A problem rarely has a single cause. A complete analysis runs three separate chains of whys:
- Occurrence: why was the defect produced? Finds the technical cause that created the problem.
- Detection: why was it not caught in time? Reveals why the controls didn't catch it sooner.
- Systemic: why did the system allow it? Exposes the deeper failure in the process or the rules.

HOW IT WORKS
- State the problem: describe it with facts — what, where, when and how much, without guessing the cause.
- Start at the top: ask the first "why?" at the highest level and answer with "because".
- Go down one level at a time: each answer becomes the next question.
- Check with "therefore": read the chain in reverse to make sure you didn't skip a step in the logic.
- Stop at the root cause: when you can no longer ask why, that's where you act with a countermeasure.
